The Dana Home Foundation Grant Recipient Final Report

Due July 15

• Download the Final Report Form.
• Complete the form.
• Save the completed report form to your computer using the filename format [Organization] - FY[grantyear] - FinalReport. (Example: HelpingSeniors_FY25_FinalReport).
• Upload the completed form by following the instructions on the Report File Upload page.

1. Download the Final Report Form by clicking the Download File link to the right. In some browsers, you may need to right click on the "Download File" link and select "Save As..." or "Save Link As..." to download a copy of the file in Word.
2. A dialog box should open giving you the opportunity to select a location to save the report template. This downloads a copy of the final report form as a Word document. 
3. Complete the fillable form and save using the naming convention above.
