The Grant Application Part I Instructions:
Read The Dana Home Foundation Program Grant Application guidelines to determine whether your organization and program meet the eligibility standards for receiving funds.Complete The Dana Home Foundation Grant Application 2016 Part I by creating a document (not to exceed 6 pages) that answers each numbered question in the order it appears in Part I of the Grant Application. (Note:  if submitting online or by email, the document must be in .pdf format.) Submit the Grant Application 2016 Part I document online, by postal mail, or by email.

Postal Mail Submission Option: Prepare a cover sheet (see sample provided on the web page: Include link here) that includes a signed statement acknowledging that the information enclosed in the application is accurate to the best of your knowledge and that you are authorized to submit this application on behalf of the organization. Grant Application Part II Instructions:

Complete The Dana Home Foundation Grant Application 2016 Part II by providing the information requested in Part II of the Grant Application. Submit The Dana Home Foundation Grant Application 2016 Part II information online or by postal mail.
 
Online Submission Option:  Combine all of the information requested in The Dana Home Foundation Grant Application 2016 Part II into one document in .pdf format. Upload this document using our submission page.

Important Note:   If you are unable to submit this information in a single document in .pdf format, you must use the Postal mail submission option below.
 
Postal Mail Submission Option:   Gather two copies of the documents requested in the Grant Application Part II. Prepare a coversheet (see sample provided on the web page: Include link here) with the following information:
  • Name of the organization (question 1 of Part I)
  • Program name (question 19 of Part I)
  • Primary contact name (i.e. a person able to answer questions and/or provide additional information about Part II of the Grant Application.)
  • Email address and telephone number of the primary contact
  • A signed statement acknowledging that the information enclosed in the application is accurate to the best of your knowledge and that you are authorized to submit this application on behalf of the organization.

Both Part I and Part II of The Dana Home Foundation Grant Application 2016  must be submitted online or postmarked by March 1, 2016.
