- Complete the application form and collect the additional, required financial information and documents.
- Save the completed application form to your computer using the naming convention OrganizationName_GrantYear_Application. (Example: HelpingSeniors_FY24_application)
- Email all required documents as listed on the application form to [email protected].

Additional Financial Information
Submit the following information as attachments with the completed application to a single email to [email protected]. Include in the subject line the name of the organization and title of the program.

- The organization’s most recent balance sheet and most recent quarterly income statement.
- The organization’s operating budget for the previous year.
- A copy of the organization’s IRS 501(c)(3) determination letter.
- Letters of support or approval from any government departments or agencies required in order to proceed with the proposed program.
Please note that the Dana Home Foundation reserves the right to request additional financial information from the applicant during the application review process.
