Dana Home Foundation Grant Recipient Report Requirements
Interim Report
- The interim report is due January 31.
- Download the Interim Report form.
- Complete the interim report form and save it to your computer using the naming convention organization_grantyear_interimreport. (Example: helpingseniors_2022_interimreport).
- Upload the completed report on the the Report File Upload page.
- You may submit additional supporting documents as necessary by emailing them to danahomefoundation@gmail.com. Be sure to include the organization name and name of the project in the Subject line.
Final Report
- The final report is due July 15.
- Download the Final Report form.
- Complete the final report form and save it to your computer using the naming convention organization_grantyear_finalreport. (Example: helpingseniors_2022_finalreport).
- Upload the completed report on the the Report File Upload page.
- You may submit additional supporting documents as necessary by emailing them to danahomefoundation@gmail.com. Be sure to include the organization name and name of the project in the Subject line.
