Dana Home Foundation Grant Recipient Report Requirements

Interim Report

• The interim report is due January 31.
• Download the Interim Report form.
• Complete the interim report form and save it to your computer using the naming convention organization_grantyear_interimreport. (Example: helpingseniors_2025_interimreport).
• Upload the completed report on the the Report File Upload page.
• You may submit additional supporting documents as necessary by emailing them to [email protected]. Be sure to include the organization name and name of the project in the Subject line.

Final Report

• The final report is due July 15.
• Download the Final Report form.
• Complete the final report form and save it to your computer using the naming convention organization_grantyear_finalreport. (Example: helpingseniors_2025_finalreport).
• Upload the completed report on the the  Report File Upload page.
• You may submit additional supporting documents as necessary by emailing them to [email protected]. Be sure to include the organization name and name of the project in the Subject line.
