Complete the Grant Application and save the completed application form to your computer using the naming convention organization_grantyear_application. (Example: helpingseniors_2022_application)
Go to the Application File Upload page and follow the instructions to submit your application.
Additional Financial Information Submit the following information as separate files using the Application File Upload page. Email questions to danahomefoundation@gmail.com.
List of Additional Files to Submit
The organization’s current balance sheet and most recent available quarterly income statements.
If the organization has been in existence for less than three years, please provide the organization’s operating budget for the previous year.
A list of the organization’s Board of Directors, if any.
A copy of the organization’s IRS 501(c)(3) determination letter, if applicable.
Letters of support or approval from any government departments or agencies needed in order to proceed with the proposed program.
Please note that the Dana Home Foundation reserves the right to request additional financial information from the applicant during the application review process.
